It is typically the quickest way to submit a complete request and helps avoid delays.
Requests must be complete and legible
Incomplete or unreadable forms may be returned and can delay processing.
Typical turnaround
Records are generally available within about 5 days from the time your request is received.
ID may be required
If records are picked up in person, bring a government-issued ID. If someone else is picking them up, that must be stated on the authorization and they must bring ID as well.
Who can sign the authorization?
The patient, parent or legal guardian, or the patient’s legal healthcare representative may sign the authorization to release records. If you are acting on the patient’s behalf, be prepared to provide supporting legal documentation when required.
