- Any X-Ray films/discs, MRI discs, CT scans, or other tests you’ve had
- Copies of all relevant medical reports (e.g., X-Ray, MRI, CT, EMG, Bone Scan)
- Referrals for treatment (if required by your insurance)
- Patient co-pay (as outlined by your insurance)
- Your referring physician’s name and phone number
- A list of current medications and any known allergies
- Completed forms (available for download):

If pre-registration is complete: Arrive 5 minutes early.
If pre-registration is not complete: Arrive 30 minutes early and bring completed forms.
Follow-Up Patients
- Please arrive 5 minutes early to complete the check-in process.

- Monday–Thursday: 8:00 AM – 4:30 PM
- Friday: 8:00 AM – 11:00 AM
After Hours Emergency Care
- 24-Hour On-Call Service: For emergencies involving our patients, call the office, and an operator will page the on-call physician.
- For life-threatening emergencies, visit the nearest emergency room or call 911.
- Note: Appointments cannot be scheduled, and prescriptions cannot be refilled after hours.

- Insurance Coverage: We accept most major insurances. Insurance acceptance can be verified when scheduling your appointment.
- Payments: Payment for services is expected at the time of the appointment unless prior arrangements have been made.
- Accepted forms of payment: Cash, Checks, MasterCard, Visa, Discover.

- Narcotic prescriptions cannot be filled over the phone, by fax, or electronically.
- All prescriptions must be picked up in-person during office hours.
- Refill Request Hours:
Monday–Thursday: 8:00 AM – 4:30 PM
Friday: 8:00 AM – 11:00 AM
- Prescription Pick-Up Hours:
Monday–Friday: 8:00 AM – 4:30 PM - Ensure timely refill requests to avoid running out of medication.
